Account Request Forms

Faculty Staff Account	This form is for requesting Email & Novell Accounts for Faculty & Staff.
Student Worker Account	Student Worker Email Accounts
Department Email Account	This form is for requesting Email Mailboxes for Departments.
Clubs & Organizations Email Account	This form is for requesting Email Mailboxes for Clubs & Organizations.
EMS Account	This form is for requesting access to the Event Managment Sytem.
Resource Account	This form is for requesting accounts to manage scheduling of rooms, vehicles and other resources.
Move Account	This form is for when you change departments at the University and need your Novell account moved to the new department.
Delete/Disable Account	This form is for when Faculty, Staff, or Students leave WSU employment. The @weber.edu Email account should be disabled/deleted.

Special Bulletin Input Form

This selection is for emergency Special Bulletin to be distributed by
email. This must be approved by a Vice President or Dean.
