Important information for Utah Residents
If you live in the state of Utah and will be using an IHC facility for your mentoring site, you will need to complete a background check, drug screen, and proof of immunizations before you will be allowed to begin your work at the facility. Contact Pat Shaw at 801-626-7989 immediately for an additional packet as this takes at least 2 months to complete.
