Forms


The following are examples of forms that can be used in the Section 504 placement process.  There are multiple examples of some form types.

These forms are taken from the Mountain Plains Regional Resource Center and Utah State Office of Educations Section 504 and Americans with Disabilities Act Handbook.  (ADA stands for Americans with Disabilities Act, of 1990, for the purpose of school issues this resource targets Section 504. 

Even though 504 forms are compatible with ADA requirements it is not the intention of this Handbook to deal with ADA.)  The following are the type of forms included.
